Implementation Plan

After the tri-annual Community Needs Health Assessment (CHNA) is completed, the CHIP identifies multiple community healthy priority areas. CHIP works with community organizations, private citizens, hospitals, schools, and businesses that are stakeholders in the health of the County to plan, develop, implement, and promote initiatives to improve the health and lives of people who live, work or attend school in Bergen County.
